The interest application deadline is before school starts in Fall. Applications are being accepted. Four (4) ways to apply. 

1. Use a QR Code Reader (app. such as Kaspersky's QR code reader and scanner. Available on: App Store and Google Playstore. ... or Snapchat) to scan the code and be directed to the online application. QR Code and the link is also available on Centennial's website.

 2. You can click on the link below to complete the digital application 

3. Download the application form-see attached file. 

4. Pick up a paper copy at the Health Academy or Centennial High School office.

TB Testing Information

An annual TB test is required for all visits to Parkview Hospital, health science clinical, and local internships. Health Academy students and staff are expected to have the test completed, along with a current flu shot. This is a policy requirement of all Pueblo healthcare organizations such as Parkview Medical Center and health care facilities. 

Please get this done before the beginning of the 2020-21 school year and retain documentation. This simple skin test can be done at your family's health care provider, the Pueblo City-County Health Department, and other local healthcare facilities. 

The TB test form is attached below. Your immunization record showing current flu shot or the paperwork from the medical facility qualifies as documentation.
